A client enquiring about the Community Connections program in 2023 shared a profound reflection that speaks to many of us: “There is a difference between being alone and feeling isolated.

The Community Connections program has been transforming lives by fostering belonging, purpose and meaningful social connections. Each client receives an individual assessment and tailored support to help them reconnect with their community.

Clients begin with one-to-one sessions with a Community Connector, reviewing a Social Navigation Plan created by their Case Manager. Connectors play a vital role in building confidence and self-esteem, supporting clients until they are ready to join group activities of interest. The program continues to grow through partnerships with local organisations offering bespoke courses and activities.

One participant shared: “It has made me believe in myself again. I always wanted to be that person who could stand up in a group and speak. This has given me the confidence and practice to do that in a safe space.

St Vincent’s Open Support also partnered with the Reginald Murphy Centre to host weekly social groups in Potts Point, offering lunch, games and conversation in a welcoming environment. Facilitated by a Community Connector and a volunteer, these sessions provide safe, supportive spaces where trust and connection have already been established.

As demand grows, the Community Connections program continues to evolve and adapt to the needs of socially isolated people, helping them rediscover community and build confidence.
